Primary Health Solutions (PHS) is a non-profit, safety-net healthcare provider serving Southwest Ohio with centers in Hamilton, Fairfield, Middletown, Oxford, Dayton, and Trenton.  Our approach to care is patient-centered.  This means that with techonolgy and our expanded resources we can, together with you, effectively coordinate your healthcare needs. Our goal is to offer a comprehensive range of healthcare services, to all members of the community, regardless of income or insurance coverage. Primary Health Solutions accepts most insurances including Medicaid and Medicaid managed care. Uninsured individuals will be seen with fees based on household income and a sliding-fee scale. Certified Application Counselors are available to help individuals with enrollment for benefits and Marketplace coverage.

The Dayton office is located in the Grafton Hill neighborhood at 300 Forest Ave. across from KHN Grandview Hospital.
