Summary
In the last decade, overall injury rates have steadily declined, but serious injuries and fatalities (SIFs) have reduced at a much slower rate or, in many cases, not at all. Despite significant advances in workplace safety tools, systems and approaches, SIFs remain a stubborn problem. This session identifies the strengths and limitations of focusing on the three major influence areas for safety— personal, social and structural—to reduce SIFs. It discusses how a 24/7 safety approach is able to better influence safety outcomes than many of the more traditional options. The presentation investigates various ways to boost employees’ personal motivation and ability in order to reduce SIFs. Attendees will leave this session with a better understanding of the major obstacles of reducing SIFs, and several practical solutions to cut down on serious injuries and fatalities.     Harmony Myers is the director of the NASA Safety Center (NSC), located at the Ohio Aerospace Institute in Cleveland, Ohio. In this role, she generates an environment of learning and information sharing; advances Safety and Mission Assurance (SMA) expertise across the agency workforce; assures consistency, effectiveness, and quality in programs, products, and services; and enhances collaborations across the agency, industry, and other government agencies. She recently served an eight-month detail as the acting deputy chief of NASA’s Office of Safety and Mission Assurance at NASA Headquarters in Washington, D.C., from August 2024 to April 2025. In this role, she was responsible for executive leadership, policy direction, functional management and coordination of agencywide program and institutional SMA activities. Previously, Myers served as the director of the Technical Excellence Office at the NSC. She was responsible for planning, organizing and directing a full range of integrated services including SMA curriculum and course development, SMA engineering support for over 2,800 NASA civil service and support contract employees agencywide, and the SMA Technical Excellence Program — NASA’s university for safety. Before taking on this role, Myers also previously served as the System Safety Technical Discipline Team Lead at the NSC. Myers began her career in 2000 at NASA Kennedy Space Center with United Space Alliance as a systems engineer for the electrical system on space shuttle solid rocket boosters. She transitioned to a reliability engineer in support of Return to Flight and became passionate about ensuring that the Space Shuttle Program returned to flight as safely and as quickly as possible. As a reliability engineer, Myers supported human spaceflight safety for the contractor’s SMA organization. She moved to NASA SMA in 2005, overseeing the contractor for Space Shuttle Program safety as a safety engineer. She went on to serve as Safety Engineering Section chief, Safety Engineering and Assurance Branch chief for the Launch Vehicle Processing Division, and branch chief of the International Space Station and Spacecraft Payload Processing SMA Division. During her career at Kennedy, Myers supported the final 36 space shuttle missions and the Ares I-X development test flight. Myers moved to NASA Headquarters in 2012 for a developmental assignment as executive director of the Aerospace Safety Advisory Panel (ASAP). She served in the role for four years, advising the NASA administrator and senior management on ASAP issues and guiding and influencing decision-making within the agency. Myers earned a bachelor’s degree in electrical engineering from the University of Central Florida and a master’s degree in industrial engineering from the University of Miami. She has been recognized with numerous leadership awards, including the NASA Space Flight Awareness Leadership Award and the Society of Women Engineers Emerging Leader Award.

Myers’ keynote will address NASA’s most significant human spaceflight accidents—Apollo 1, Challenger, and Columbia—and the ways they have shaped the way the Agency thinks about safety, leadership, and organizational culture. She’ll walk through the warning signs that preceded each accident, what the investigations uncovered and the recurring themes that emerged across all three events. These tragedies were defining moments for NASA. They pushed the Agency to take a hard look at communication, decision making and risk management. More importantly, they led to lasting changes in how we build a strong safety culture and strive for organizational excellence. Today, the Safety and Mission Assurance community plays a key role in making sure those lessons stay front and center, working to create an environment where concerns are heard, accountability is clear, and continuous improvement is part of NASA’s DNA. Myers will share how NASA’s past has helped build a safer, smarter future as well as what other organizations can learn from the journey.
